About Supports Coordination
Supports coordination is the initial and ongoing link to resources and resources within the mental retardation system.
 
Supports coordinators are an integral team player and a leader in locating, coordinating, and monitoring the identified services for the consumer.
 
Support Coordinator’s Functions:
  • Needs Assessment:
    In cooperation with the family and the individual, the Support Coordinator will assess the needs for new services or changes in services. The PUNS document will be reviewed at least annually. There will be dissemination of information and support to individuals and others responsible for the planning and implementation of services.
     
  • Annual Plan Development:
    There is development of an annual Support Plan. Plans are developed through team meetings, information gathering throughout the year, and consultation with all team members. The plan is to identify ways to meet the service needs of the individual. The plan is then entered into the OMR web-based data system, HCSIS. (Home and Community Services Information System) Reviews and revisions are completed in cooperation with all team members.
     
  • Locating and Coordinating Services:
    The Support Coordinator provides assistance to the individual and family in linking, arranging for, and obtaining services specified in the individual’s plan including medical, social, habilitation, education, or other community services that the individual needs to live at home and in the community. They also coordinate the management of services and supports stipulated in the plan in cooperation with the individual, family and providers of service.
     
  • Monitoring of Services:
    The Support Coordinator conducts regular visits to ensure that services are being provided as per the Individual Support Plan. They are also responsible for following up on any health, safety or other concerns identified at the time of the visit, by telephone calls or as identified in Incident Reports. The Coordinator is to assure the individual is receiving the appropriate quality, type, duration and frequency of services and benefits.
