Contact the Intake Coordinator at (610) 278-1284 for an initial screening and to receive an intake packet. The intake packet will include information regarding services provided under the Office of Mental Retardation, a list of required documents to determine eligibility, as well as other resource information that may assist in supporting the individual with a diagnosis of Mental Retardation.
Upon review of the required documentation and if eligibility has been determined, an intake interview will be scheduled. A Service Needs Assessment will be sent to you to be completed prior to the intake interview. At the intake appointment, the Intake Coordinator will review the Service Needs Assessment with you and determine whether any additional evaluations are needed. Individual and family resources will be reviewed, as well as generic and specialized mental retardation services. At this time, the Intake Coordinator will discuss with the family and the individual how best to meet the needs identified.
After the intake appointment, the Intake Coordinator will assign the case to a Supports Coordinator who is located in one of our three local offices. This is based on the school district in which the individual resides. Montgomery County has three Supports Coordination offices within Montgomery County.
