First, there are certain eligibility requirements that must be met to be “opened” into the system. These requirements include:
-A psychological evaluation from a licensed psychologist, certified school psychologist, physician, or psychiatrist with full scale I.Q. results (70 or below) and a diagnosis of Mental Retardation
-Onset of Mental Retardation prior to the individual’s 22nd birthday
-Significant limitation in adaptive functioning in at least 2 of the following areas: communication, self-care, home living, social/interpersonal skills, use of community resources, self direction, functional academic skills, work, health, and safety.

After an initial screening, you will be provided with a packet of information regarding services provided and a list of required documents to determine eligibility (if applicable).
If the person with disabilities meets the eligibility requirements and is “opened”, the Intake Coordinator will assign the individual to a Supports Coordinator from one of three local offices. The office assignment is based on the school district in which the individual resides.
