Montgomery County, PA

-   A psychological evaluation from a licensed psychologist, certified school psychologist, physician, or psychiatrist with full scale I.Q. results (70 or below) and a diagnosis of Mental Retardation

-   Onset of Mental Retardation prior to the individual’s 22nd birthday

-   Significant limitation in adaptive functioning in at least 2 of the following areas: communication, self-care, home living, social/interpersonal skills, use of community resources, self direction, functional academic skills, work, health, and safety.

If you are interested in Supports Coordination for you or someone you know, contact Dina Scarci, Intake Coordinator, at 610.278.5666. After an initial screening, you will be provided with a packet of information regarding services provided and a list of required documents to determine eligibility (if applicable).

If the person with disabilities meets the eligibility requirements and is “opened”, the Intake Coordinator will assign the individual to a Supports Coordinator from one of three local offices. The office assignment is based on the school district in which the individual resides.
